Update on PEPFAR’s Programming Budget for 2024/2025

Since the start of the U.S. President’s Emergency Plan for AIDS Relief (PEPFAR) in 2003, the program has utilized multi-year appropriations.  The Biden Administration has every year requested at least $4.7 billion for the PEPFAR program in its annual budget request to Congress, on top of funding for the Global Fund to Fight AIDS, Tuberculosis and Malaria — demonstrating unwavering commitment to fighting HIV/AIDS globally.  These funding levels, coupled with multi-year availability, have enabled the program to have an operational budget of more than the annual appropriation when prior-year funds remained available.  PEPFAR has used this strategy successfully to accelerate progress in the fight against HIV/AIDS.  However, balances from prior years have decreased, while the PEPFAR annual appropriation level has remained relatively stable for the last decade, consistent with levels requested by the Biden Administration that align to achieving PEPFAR’s goals.  

As funding levels adjust in response to the decrease in prior year balances, PEPFAR will continue to utilize a data-driven approach to identify gaps as defined in our five-year strategic plan, particularly among the key and priority populations most at risk for HIV/AIDS.  Based on this approach, overall average planned programmatic funding reductions from fiscal year 2024 to 2025 will be lower for those activities targeting key populations, compared with reductions in overall program funding for PEPFAR-supported countries and regions.  

To ensure stability in PEPFAR’s provision of life-saving services and progress towards reaching PEPFAR’s goals, the program holds a modest amount of funding to address unanticipated contingencies in partner countries. Each year, in collaboration with country partners, multilateral partners, and civil society, PEPFAR utilizes data to plan and implement programs in support of our goal of ending HIV/AIDS as a public health threat by 2030. 

Strong bipartisan support from 11 Congresses and four Administrations and the generosity of the American people have been central to the success of this collaborative approach – which has saved more than 25 million lives, enabled 5.5 million babies to be born HIV-free, and improved the lives of millions more.  The United States, through PEPFAR, remains steadfast in its commitment to the communities hardest hit by HIV/AIDS and to working with partners across sectors to align resources to areas of greatest need and in order to achieve the greatest impact.
